Chief complaint: Patient has known h/o T2DM since 2005. Due to the T2DM problem, he feels with low energy levels.
Presenting illness: A Patient has been suffering of symptoms and signs associated with T2DM. Patient is taking medication for T2DM since 2005, But has got good control over BSL level since 2014. Patient is taking the following medications for Rx and management of T2DM.
