chief complaint: c/o dimness of vision in both eyes since the age of 8 years.

presenting illness: a 26 years old male presented with history of dimness of vision in both eyes since age of 8 years old, he has got a loss of vision in both eyes. conservative medical treatment is going on.

patient overall health is good but eyesight is not good. he can see moving hands from 1 meter long in daylight but can’t recognize how many hands are there. he can’t read books or text on the screen and he can’t even recognize people near him, he only sees their forms.
