CHIEF COMPLAINT: c/o raised blood sugar levels & weight loss due to T2DM since 2017. C/o jaundice with yellow discolouration on nails & sclera of both eyes since last 3-4 weeks.

PRESENTING ILLNESS: A 53 year old male k/c/o T2 diabetes with gradual loss of weight since 2017. Conservative medicinal treatment is going on. Now Patient has come for getting his T2DM treated by experimental stem cell therapy. He has been explained about status of stem cell therapy in India about experimental nature & he has agreed voluntarily to undergo this experimental stem cell therapy.

Written informed consent was taken and the same has been explained in their understandable language by treating stem cell therapy consultant.
